Go to Dwarf Chapter 3, the camp at Komat in Mount Bloodhorn and talk to Hammerstriker Hasher there, He will give you three sacks of rotting fruit. There are three Slop Baskets Around 23k X 24k. Use the Slop Basket to put fruit from one sack in it. Kill the attacking Carrion seeker vulture after using each basket. If all went right, you'll have an empty burlap-sack, that smells of rotten fruit, instead of the sack of rotting fruit in your backpack. Go back to the Hammerstriker Hasher and talk to him once again.

The chest will be opened and reveal a smaller chest inside and you will get the following text: You now see that the shape of the talisman matches the shape where the initials are engraved. As you apply the talisman to the strongbox, the chest opens with a magical burst.
Click on the smaller "Battered Strongbox" to receive a "Bejewelled Skull". Click on the "Bejewelled Skull" in your inventory to spawn Cihtym, a level 39 Champion Bloodletterand see the following text:
Deciding to tinker with the skull, you knock out the gems from the eye sockets. Kill Cihtym to receive the unlock.
